Schedule

1. The training program and objects

The 3-year program for oral and maxillofacial specialty offers to dentist with national license and 2-year experiences of general dental practice. The permission was via an entrance exam. The goals of the specialty program are mainly basic research combined with clinical training. Teaching ability was an addition. The trainee will get the specialty license after finishing the program.

2. Aims

(1)  The first one and half an year the participant is under the basic training at both OPD and dental ward. The trainee will be expected to become a good assistant in a complicated major surgery, a well-trained surgeon for dental implants and minor dentoalveolar surgery. The ability for taking care of inward patients is also prepared.

(2)  The next half and year, the trainee is asked for comprehensive medical training programs in other department. During the period, the research and the treatise should be accomplished.  

(3)  At the third year, the trainee should prepared himself in charge of major oral maxillofacial surgery. The talents of teaching, researching, leading ability become mature, too.

3. Requirements of the training program

(1)  The basic requirement

The basis for a oral maxillofacial surgeon should know well in erupted tooth extraction, odontectomy of impaction such as upper and lower 3rd molars, mesiodens or impacted maxillary canines, minor dentoalveolar surgeries, the diagnosis of oral maxillofacial diseases. A good surgeon should also be familiar to records of physical examinations, consultations, pre-operative and post-operative general care of inward patients, aspetic procedures and instruments in operation rooms.

The minimal requirements for dentoalvleolar surgery are following:

Patients undergo odontectomy, pre-prosthetic surgery, oroantral or oronasal communications or neuropathic pain under general anesthesia

Orthognathic surgery, temporomandibular joint diseases, cleft lip, alveolae, or palate, dental implantation or oral facial reconstruction

(2)  Duty time in nighttime and holidays

Otherwise, the trainee is in the duty course of inward patient care at nighttime and holidays. Proper management for emergent cases is also responsible. The on-duty doctor is asked to realize the history, present illness and treatment course of each inward patient, and take care them accurately under the medical order. In the duty time, the doctor in charge should check the patient conditions at least twice a day, usually between patient getting up and going to bed. For patients with emergency, the on-duty doctor are asked for the first-lined management, and making contact with the chief resident or visiting staffs for further management, if any difficulties.

(3) Educational conferences

 

Except participating the clinical practice in dental departments and operation rooms, conferences are held in order to polish the trainees’ academic knowledge and to develop a critical thinking. Trainees are expected to solve problems and do researches independently.  The scheduled conferences are as follows:
(1)  Dental inpatient conference
(2)  Literature review seminar
(3)  Combined meeting of OMS and prosthodontics
(4)  Combined meeting of OMS and orthodontics
(5)  Combined meeting of OMS and pathology
(6)  Nationwide seasonal clinical case conference
(7)  Nationwide surgical seminar
(8)  Medical round of the chief resident and ward director

(4) Advanced training of OMS

The trainees major in not only the practice related to oral and maxillofacial surgery, but also the basic training in other medical departments.  The following departments are the most common training units.
(1)  Anesthesiology: to learn the management of airway
(2)  Plastic surgery: to learn the head and neck reconstruction and wound care
(3)
 
ENT: to learn the anatomical structures and functional physiology adjacent to the oral cavity
(4)  Internal medicine: to learn the basic medical knowledge
(5)  Emergency: to learn the first-aids
(6)  Intensive care unit: to learn the intensive care of inpatients

As in OMS departments, the trainees are expected to improve the following skills and to accomplish the requirements of OMS specialty:
(1)  OPD and ward course
    
- Minor OMS surgeries
    
- Odontectomy of impacted teeth
    
-  Pre-operative preparation
    
-  Post-operative care
     
-   Inpatient managements

(2)  OR course
    
-  Basic principles of surgeries
    
-  Odontectomy of impacted teeth
    
-  Cyst enucleation (with/without apicoectomy) and marsupialization
    
-  Debridement of head and neck infection
    
-  Reduction of the facial bone fracture and soft tissue management
    
-  Pre-prosthetic surgeries
    
-  Dental implantation
    
-  Excision of benign tumors
    
-  Excision of malignant tumors
    
-  Orthognathic surgeries
    
-  Head and neck reconstructions
    
-  Surgeries of TM joints

(3)  ER course
    
-  Assist the duty residents
    
-  Emergent operations

(5) Researches

Individual researches under the guidance of the senior visiting staffs are encouraged.

The trainees are expected to propose a hypothesis, draw a project, analyze the results and finish a thesis in their training course. A publish on the Chinese Journal of Oral and Maxillofacial Surgery is also required for the training of a specialist.
